My Food Choices
If your TLC Program includes Therapeutic Nutrition for weight loss or for reversing chronic disease, your Assessments include the calculation of your Whole Plant Food (WPF) Score. (If not, you may skip this page and go to My Results.)
Before you Assess your Food Choices using the WPF Survey at the end of the week, you may want to use: a) Forms 1 & 2 below, b) Only Form 2 below, or c) Neither Form 1 or 2 below. These two forms are designed to help you track your Food Choices during the week for a more accurate Nutrition Assessment at the end of the week. Most participants start by using both Forms 1 & 2, but then eliminate the forms sequentially as they learn how to track their food choices more easily - but still accurately.
1. Use the 24/7 Nutrition Log to record ALL the foods you eat and ALL the beverages you drink (including water) at each meal and in between around the clock, 24/7.

2. At the end of the week, you can use your 24/7 Nutrition Log (above) to fill-out the Food Category Log (below), OR you can use the form below to directly record your food and beverage choices during the week.

3. Using your Food Category Log (above) take the Whole Plant Food (WPF) Survey (linked below) to calculate your WPF Score. Record your answers on your WPF Scoresheet to track your dietary changes throughout your TLC Program.
