Intake Tests & Measurements

In order to personalize the Best Weigh Program to meet your individual needs and ultimately achieve your personal nutrition and weight loss goals, you will need to meet with your personal OptiHealth Coach on two consecutive Sundays one week apart.

Your Coach will explain and help you complete the necessary worksheets for participating in the Best Weigh Program. If your Coach has not contacted you yet to schedule your appointments, you may contact your Coach anytime.

Do NOT print and try to complete these worksheets. Your Coach will walk you through the intake process and will fill-in most of the information for you. You will be provided copies.
